Immunization Record Form Due

The Immunization Record Form is due October 1 for all incoming summer/fall students. You can submit your form via mail, fax, email or in-person to the Immunization Office. Form and details about the immunization requirements is available here:

Virginia state law and George Mason University policy requires all students to be immunized according to the Centers for Disease Control and Prevention (CDC) guidelines. Student Health Services is responsible for collecting and maintaining students’ immunization records.
